This video demonstrates a wrist arthroscopy for a distal radius mal-union, initially considering a possible open osteotomy. The procedure begins with insufflation of the wrist showing promising resistance for mild pathology, followed by the introduction of the arthroscope where significant fraying of tissues is noted immediately.
It highlights the debridement of frayed tissue and synovitis, and examines an intra-articular step off from the mal-union distal radius fracture. Because the step off measured no more than 2mm and was deemed acceptable, an osteotomy was not performed. The video then showcases a thermal shrinkage capsulorrhaphy and a switch to the mid-carpal portal, confirming that while x-rays showed SL widening, the SL ligament remained intact, before concluding with further debridement of mid-carpal joint synovitis.
